- W997639579 abstract "Objective To analysis the position of femoral implants after hip revision using extended trochanteric osteotomy (ETO), and evaluate the clinical significance of ETO in hip revision. Methods From January 1998 to June 2007, 33 patients (33 hips) underwent total hip revision using ETO performed, which included 19 males, 14 females, with the mean age of 62.8 years. To analyze the complication rate such as trochanteric osteotomy nonunion, broken wires, the trochanterie migration, the stem subsidence, and the posi-tion change of the femoral stem. The hip functions were evaluated with Harris score and WOMAC score sys-tems. All measurements were done from the supine anteroposterior (AP) radiograph of the pelvic and a Lauenstein lateral radiograph. Results Of the 33 patients (33 hips), 25 patients were followed up for an av-erage of 63 months. The osteotomy fragment was attached with the use of 3-5 cerclage wires. The mean Har-ris score improved from 38.4 preoperatively to 88.7 postoperatively, and the mean WOMAC decreased from 56.2 preoperatively to 42.8 postoperatively. The average time for the healing was 5 months (4 to 10 months).None of the cerclage cables was broken. Three femoral components were in a condition of subsidence 3.4 mm on average (3-7 mm) in the 6 months postoperatively; the union occurred within 6 months without any further migration. Two patients had undergone the femoral stem position change (one varus, another valgus).No trochanterie fragment fracture occurred during operation. No late fracture, wires broken, infection, osteol-ysis, or heterotopic ossification was found postoperatively. One patient had hip dislocation 'after operation. No other complications occurred. Conclusion The ETO can facilitate removal of the solidly-fixed femoral im-plants, allow reliable reattachment of trochanteric fragments and achieve excellent healing and implant stabil-ity in the revision total hip arthroplasty. However, the femoral stem subsidence, position change are the main complications of this procedure.Key words: Arthroplasty, replacement, hip; Reoperation; Osteotomy" @default.
